The purpose of this questionnaire is to gather information for the Breeders Directory. Although similar to the information page for the Free Member Website, there are some subtle differences. (Be sure to complete the Member Web Page Questionnaire in order to qualify for the free one-page web site.)

IMPORTANT -- in order to be listed in the Breeder's Directory, you must also
agree to the Code of Ethics. Click the link to complete that form online.

You have the option to restrict the amount of data found in the directory. Only the items in bold print are required -- those in regular print are optional.
